Complete an extensive personal history intake form prior to your appointment and you will be provided with support, guidance and recommendations on herbals, supplements, or minerals, or lifestyle changes to start your wellness journey. Nutrient Survey
Complete and submit the Nutrient Survey Questionnaire via email. Your answers will be evaluated and you will receive a customized report ranking 28 areas of your body including vitamin and mineral deficiencies, major body system malfunctions, Candida and parasite involvement, and even fat utilization. For areas of your body that show severe or moderate problems a list of herbs and vitamins which have been shown to improve those particular areas will be included in your Nutrient Survey report.
